Downloadable Forms:

PDF forms must be downloaded, completed, and submitted via email or in person as directed.

  • Ensure all required fields are completed before submission.
  • Send completed forms to the appropriate department via the email provided in each section.

Notarized Documents

  • Applications requiring notarized documents can be submitted electronically as long as the Notary Signature, Date, and Seal are clearly visible in the upload.

Payment Information

  • In-Person Payments:
    For PDF-based forms, follow the payment instructions provided by the relevant department after submission.
